each Subcontractor who will perform work or labor or render service to the prime
CONTRACTOR in or about the construction of the work or improvement, or a Subcontractor
licensed by the State of California who, under subcontract to the prime CONTRACTOR,
specially fabricates and installs a portion of the work or improvement according to detailed
drawings contained in the plans and specifications, in an amount in excess of one-half of one
percent of the prime contractor's total bid or, in the case of bids or offers for the construction of
streets or highways, including Midges, in excess of one-half of one percent of the prime
CONTRACTOR'S total bid or ten thousand dollars ($10,000), whichever is greater. The Bidder
shall list the Subcontractor's License Number and, in accordance with Labor Code Section
1725.5 and 1771.1, the Subcontractor's Department of Industrial Relations Registration
Number. The Bidder shall also list the portion of the work that will be done by each
Subcontractor under this Contract. The prime CONTRACTOR shall list only one SUB-
CONTRACTOR for each portion as is defined by the prime contractor in his or her bid- Failure to
list subcontractors with portions of the Work in excess of the minimum amounts listed above
constitutes a declaration by the CONTRACTOR that it is qualified and able to self -perform the
work. Any CONTRACTOR found to be unqualified or unable to self -perform any part of the
Work not listed below may be disqualified. The Bidder's attention is directed to the provisions of

base maximum depth of 60", fittings, all horizontal and vertical adjustments as necessary to
install and connect new meter to the existing water service on both sides of the meter, tracer
wire, exposure of the water service valve to determine service depth and orientation, bedding,
backfill, compaction, testing. All piping, fittings, and meters to be disinfected and visually tested
for leaks prior to backfill.
Bid Item 4 shall include the complete installation of one meter on the exisfirig single service
valve or the complete installation of one meter on an existing double service connection of
which the other half of the existing double service connection does not receive a new meter. Bid
Item 5 shall include the complete installation of one meter on one side of a double service
connection; Bid Item 5 is applicable to installations including two meter installations Within a
single excavation.
Ali meter installations shall include the installation of a valve box for each water service Valve.
The Valve box may be new (as provided by the District at the identified staging area shown on
Plan Sheet G3) or an existing valve box may be reinstalled at the direction of the Districfs field
representative. The Contractor is responsible for the removal from the project area and disposal
of existing valve boxes not reinstalled. Valve boxes may be Christy F08i G05� B09, B1017 or
e�qual.
Addonal bid items are detailed below which will be used to compensate contractor for various
additional installation conditions, where applicable and approved by the District, including traffic -
rated installations, patch paving, groundwater dewatering, additional excavation depth, service
valve replacement, and unknown conditions.

sides of the meter connection. Excavations deeper than 76" shall be paid for on a time -and -
materials basis under Bid Item 17, Contingency Work.

protection of life and limb, planning, designing, engineering, furnishing, constructing and
removing temporary sheeting, shoring and any other work necessary to conform to the
requirement of any permits, OSHA and the Construction Safety Orders of the State of California,
pursuant to the provisions of Section 6707 of California Labor Cade, and all labor, tools,
materials and appurtenances complete in place per the plans and specifications.
Bid item shall include additional labor, materials, tools, equipment and incidentals, to connect
one or both sides of the new water meter to the deeper existing water service line. Bid item to
include all fittings & pipe required to make up the vertical change in elevation between the new
meter and the existing curb stop and the existing service line going to the property. Bid Item
shall include excavation to the depth of 76", sand bedding, new aggregate base or native
backfifl (as required by plans & specifications), transporting materials, tracer wire, compaction,
disinfection, protectionfrepair of existing services and utilities encountered during installation,
complete and in place. All piping, fittings, and meters to be disinfected and visually tested for
leaks prior to backfill.

Bid Item 17 - Contingency Work - Paid for on a Time -and -Materials Basis: The District has
included a lump sum amount on the bid form to compensate the Contractor on a Time and
Material Basis, only upon the approval of the Engineer, for work completed to address certain
variations in field installation conditions which have not been adequately or specifically identified
in the plans and specifications provided to the Contractor for bidding, or provided for in other bid
items above. These items may include but are not limited to the following:
I ) Rock excavation and removal from site;
2) Excavation below 76" depth;
3) Tree protection per Detail 41D2 of the Plans;
4) Replacement of private improvements at the direction of the District;
5) Installation of sewer riser and sewer G5 box at uncovered clean outs;
6) Expanded excavation and other work to accommodate an extended install location
based on field conditions, beyond the 36" horizontal distance included in the base
meter installation bid item, and shown on Detail VD1 of the Plans;
7) A/C patch paving thicker than 3",
8) Seal coat;
9) Concrete driveway restoration;
10) Replacing the service line from the service valve back to the main; and
11) Other additional work approved by the District.
Contractor shall draw upon the allotted amount through the submittal and District -approval of
documented costs for materials, equipment, crew and subcontractors as per the specifications,
specifically Section 00 72 00 Part 2.10, Extra Work. If the total amount is exceeded as the
project progresses, the additional time and material costs will be addressed as a changed
condition. Time and material work under this item shall include all work required to address the
unknown conditions. The District shall not be obligated to pay Contractor the entire allotted

`2.10 CULTURAL RESOURCES
A. If required as a condition of the District's contract for funding, the Contractor
shall abide by the following:;
1. If archaeological features or materials are unearthed during any
phase of project activities, all work in the immediate vicinity of the
find shall halt until Recipient has contacted the State and the
significance of the resource has been evaluated. Any mitigation
measures that may be deemed necessary must have the approval
of the State, and shall be implemented, pursuant to the Secretary
of the Interior's Standards and Guidelines for Archaeology and
Historic Preservation, 48 CFR 44716, by a qualified archaeologist
representing the Recipient prior to the resumption of construction
activities.
2 If human remains are exposedby activity related to the project,
the Recipient must comply with California State Health and Safety
Code, Section 7050.5, which states that no further disturbance
shall occur until the County Coroner has made the necessary
findings as to origin and; disposition pursuant to California Public
Resources Code, Section 5097.98.
