TASK ORDER NO. 24 CONSULTING SERVICES This Task Order is made and entered into as of the 30th day of September 2025 by and between South Tahoe Public Utility District, hereinafter referred to as "DISTRICT" and Lumos and Associates, Inc., hereinafter referred to as "CONSULTANT". In consideration of the mutual promises, covenants, and conditions as addressed in the Master Agreement dated November 21, 2024, and as specifically hereinafter set forth, the parties do hereby agree as follows: 1. PURPOSE The purpose of this Task Order is to authorize and direct CONSULTANT to provide professional surveying services to support District's operations at Harvey Place and Indian Creek Reservoirs in and around Markleeville, Alpine County, California. 2. SCOPE OF WORK The scope of work for these services is further described in CONSULTANT's Proposal dated September 19, 2025. Said Proposal is incorporated herein as Exhibit "A" and by this reference is made a part of this Task Order. 3. STAFFING Staffing will consist of Greg Phillips, P.L.S., WRS, Surveying Division Director, Project Principal; Michael Craven, P.L.S., P.E., Surveying Group Manager; and Greg Lindsey, Project Coordinator. CONSULTANT shall not change staffing without approval by DISTRICT. Any changes shall provide for equivalent qualifications as far as possible and practical. 4. TIME OFPERFORMANCE Tasks for these professional services will commence on verbal notice to proceed from Adrian Combes, Director of Operations for South Tahoe Public Utility District on or about October 1, 2025, and shall be completed on or about December 1, 2025. 5. PAYMENT Payment to CONSULTANT for services performed under this Task Order shall be as provided in Article IV of the aforementioned, Master Agreement and shall not exceed the amount of Nine Thousand Five Hundred Dollars ($9,500) unless the scope of work is changed, and an increase is authorized by DISTRICT. 1 6e INSURANCE COVERAGE CONSULTANT's Errors&Omissions coverage shall be$1,000,000 aggregate with a deductible of not more than $100,000 for the duration of this Task Order. 7. EFFECTIVE DATE This Task Order No. 24 will become effective upon execution by both parties and shall serve as a Notice to Proceed upon execution by DISTRICT. (Lumos) appreciates the opportunity to submit this proposal to provide professional surveying services to support your operations at the Harvey Place and Indian Creek reservoirs in Alpine County. It is our understanding that both dams (and their auxiliary dams) have been monitored for settlement periodically over their lifespan. Monuments were established when the dams were constructed and Lumos has been provided with the most recent survey results and corresponding data spreadsheet. Based upon review of the data, only elevations of the monuments have been tracked and no data was provided on the horizontal locations of the monuments. It is our understanding that based on the previously observed settlement,the frequency of monitoring surveys needs to be every 6 months. We propose to provide an initial task for the first round of monitoring that allows for site familiarization and gaining a better understanding of the prior survey work. Once we have completed the initial round, Lumos will prepare a fixed fee for each subsequent monitoring effort. Task 100 —Initial Site Visit&First Monitoring Lumos will review the plans provided and survey data and develop a survey plan. A crew will be dispatched to recover the existing monuments and measure their elevations relative to project benchmarks. The elevations will be established using a digital level. Lumos will reduce the data and update the provided spreadsheet. The results of the survey will be documented in a memo to the district and stamped by a California Licensed Land Surveyor. Fees Due to the uncertainties in the first round of monitoring, a T&M based approach will be most equitable for all parties. Therefore, Lumos will conduct work on this task on a T&M basis in accordance with our current fee schedule (attached) up to a limit of$9,500. September 19,2025 Page 2 LA25.698 Fees will include El Dorado County prevailing wage rates paid to all field crew members for time on site. The hourly rate for a 2-person prevailing wage field crew is: $395 / hour. Schedule Lumos is prepared to begin work on this project within 2-3 weeks of our notice to proceed. We anticipate needing an additional 2 weeks to complete the field survey, reduce the data, and prepare the results. Thank you again for allowing Lumos&Associates to provide you with this proposal.
