TASK ORDERNO. 20
CONSULTING SERVICES
This Task Order is made and entered into as of the 7th day of March 2025 by and between
South Tahoe Public Utility District, hereinafter referred to as "DISTRICT" and Lumos and
Associates, Inc., hereinafter referred to as "CONSULTANT".
Inconsideration of the mutual promises, covenants, and conditions as addressed in the
Master Agreement dated November 21, 2024, and as specifically hereinafter set forth, the
parties do hereby agree as follows:
1. EURPOSE
The purpose of this Task Order is to authorize and direct CONSULTANT to provide
professional surveying services to support operations for DISTRICT's Underground.
Repair-Water and -Sewer crews' Works located in the District's service area in the
City of South Lake Tahoe, and unincorporated areas of the County of El Dorado,
California.
2. SCOPE OF WORK
The scope of work for these services is further described in CONSUL_TANT's
Proposal dated March 4, 2025. Said Proposal is incorporated herein as Exhibit"A"
and by this reference is made a part of this Task Order.
3. STAFFING
Staffing will consist of Greg Phillips, P.L.S., WRS, Surveying Division Director,
Project Principal; Michael Craven, P.L.S., P.E., Surveying Project Manager; Jason
Tokheim, Assistant Project Manager; and Greg Lindsey, Project Coordinator.
CONSULTANT shall not change staffing without approval by DISTRICT. Any
changes shall provide for equivalent qualifications as for as possible and practical.
4. TIME OF PERFORMANCE
Tasks for these professional services will commence on verbal notice to proceed
from Adrian Combes, Director of Operations for South Tahoe Public Utility District on
or about March 25, 2025, and shall be completed on or about October 15, 2025.
5. PAYMENT
Payment to CONSULTANT for services performed under this Task Order shall be as
provided in Article IV of the aforementioned, Master Agreement and shall not
exceed the amount of Fifty Thousand Dollars 50,000 `unless the scope of work is
changed, and an increase is authorized by DISTRICT.

6. INSURANCE COVERAGE
COPJvULTANT'o Erroro&Omieeions coverage shall hP$1,nnn,nnnaggregate with
a deductible of not more than $100,000 for the duration of this Task Order.
7. EFFECTIVE DATE
This Task Order No. 20 will become effective upon execution by both parties and
shall serve as a Notice to Proceed upon execution by DISTRICT.
IN WITNESS WHEREOF,this Agreement is executed by the General Manager and
Clerk of the Board of the South Tahoe Public Utility District, and CONSULTANT has
used this Agreement to be executed the day and year first above written.

Monument Preservation On-Call Services,El Dorado County, California
Dear Adrian:
Lumos & Associates, Inc. (Lumos) appreciates the opportunity to submit this proposal to provide
professional surveying services to support your operations. It is our understanding that El Dorado
County and the City of South Lake Tahoe are going to be requiring compliance with the CA Business
and Professions Code Chapter 15 (Land Surveyors Act), Section 8771 going forward. Compliance
requires pre and post construction monument preservation certificates and associated survey work.
Based upon our understanding we propose to provide the following scope of service.
Task 100—Monument Preservation On-Call Services
Lumos will coordinate weekly with the district on upcoming projects that require monument
preservation work and an order of priority. Once a site is released there are four phases of work
that can occur, depending on if a monument is found and if a found monument is disturbed during
construction:

Phase 1—Determination of existing monuments.
a. Lumos will research record mapping and deeds in the area identified.
b. Lumos will send a survey field crew to search for existing monuments.
c. If no monuments are found, the certificate will be filled out as such. If needed a
letter can be prepared for submittal to El Dorado County or the City stating no
monuments found within the area.
Phase 2 Location of existing monuments—if present.
a. If existing monuments are found during Phased, Lumos will locate said monuments
together with accessory monuments in the area. Monuments will be flagged in the
field to avoid disturbance during construction.
b. Lumos will prepare a corner record for submittal to the County in conformance with
the California Land Surveyors Act, showing the location of the monuments and its
accessories.

Phase 3 —Verification of Monuments after Construction.
a. Lumos will make a return field visit after construction to verify the monument had
not been disturbed during construction.
b. If the monument is found undisturbed, the certificate will be filled out as such. If
required a letter can be prepared for submittal to El Dorado County or the City
stating all monuments remained undisturbed post-construction.
Phase 4 Re-establishment of missing or disturbed corner.
a. On the return visit, if monuments were disturbed or removed, Lumos will replace the
monaimrnt with a now one in conformance with the California Land Surveyors Act.
b. A final Comer Record will be filed with the County Surveyor showing the
replacement of the disturbed monument.
Pre and post construction certificates of monument preservation will be completed accordingly. All
work will be performed under the supervision of a California Licensed Land Surveyor, No. 9685.
Fees
Due to the unknown nature of what will be found in the field and how many phases of work will be
required at each comer, a T&M based approach will be most equitable for all parties. Therefore,
Lumos will conduct work on this task on a T&M basis in accordance with our current fee schedule
(attached) up to a limit of$50,000 at which point the performance of the monument preservation
process will be evaluated with STPUD to determine future work authorizations.
Fees will include El Dorado County prevailing wage rates paid to all field crew members for time on
site. The hourly rate for a 2 person prevailing wage field crew is: $395/ hour.
Schedule
Lumos is prepared to begin work immediately on this project. We ask that a minimum 3 working
day notice be provided before the pre-construction work needs to be completed. However,we
understand that due to emergency situations this may not be possible and we will attempt to
accommodate monument preservation needs as quickly as we can.

These rates apply to services rendered through December 31,2025.Services provided after
this date will be invoiced according to the Standard Fee Schedule in effect at that time.
