TASK ORDER NO. 19 CONSULTING SERVICES This Task Order is made and entered into as of the 15th day of October 2024 by and between South Tahoe Public Utility District, hereinafter referred to as "DISTRICT" and Lumos and Associates, Inc., hereinafter referred to as "CONSULTANT". In consideration of the mutual promises, covenants, and conditions as addressed in the Master Agreement dated November 20, 2014, and as specifically hereinafter set forth, the parties do hereby agree as follows: 1. PURPOSE The purpose of this Task Order is to authorize and direct CONSULTANT to provide professional services to perform a topographic survey for DISTRICT's Park Avenue Waterline Replacement Project located in the City of South Lake Tahoe, County of El Dorado, California. 2. SCOPE OF WORK The scope of work for these consulting services is further described in CONSULTANT's Proposal dated October 9,2024. Said Proposal is incorporated herein as Exhibit "A" and by this reference is made a part of this Task Order. 3. STAFFING Staffing will consist of Greg Phillips, P.L.S., WRS, Surveying Division Director, Project Principal; Michael Craven, P.L.S., P.E., Surveying Project Manager; Mitch Bartorelli, Photogrammetry Manger; and Rick Byrem, P.L.S., WRS, Surveying Project Manager. CONSULTANT shall not change staffing without approval by DISTRICT. Any changes shall provide for equivalent qualifications as far as possible and practical. 4. TIME OF PERFORMANCE Tasks for these professional services will commence on October 15,2024, and shall be completed on or before February 15, 2025. 5. PAYMENT Payment to CONSULTANT for services performed under this Task Order shall be as provided in Article IV of the November 20, 2014, Master Agreement and shall not exceed the amount of Thirty-Eight Thousand Dollars ($38,000) unless the scope of work is changed, and an increase is authorized by DISTRICT. 1 6. INSURANCE COVERAGE CONSULTANT's Errors& Omissions coverage shall be $1,000,000 aggregate with a deductible of not more than $100,000 for the duration of this Task Order. 7. EFFECTIVE DATE This Task Order No. 19 will become effective upon execution by both parties and shall serve as a Notice to Proceed upon execution by DISTRICT. We estimate the length of Park Avenue project to be approximately 6,500 linear feet. Project Understanding It is our understanding that the scope of work for this project is to prepare a topographic survey map for waterline design purposes. The limits of the project are per the attached exhibit prepared by the District that generally include all roadways within the project area and extending 5' behind existing right of ways. In addition, there is a location on the Park Ave. project that is outside of existing street right of way. We propose the following Scope of Services for the project. Project Scope Topographic Mapping Lumos will meet on-site with district representatives to gain a better understanding of the project and survey requirements. During the meeting, Lumos will also work with the district to understand which survey monument locations are likely to be disturbed during construction and would need to be searched for during the topographic survey phase of the project. Once we have a greater understanding of the district's specific needs from the site meeting, our methodology will include a fixed wing flyover of the site to collect aerial imagery from which an orthophoto of the project area will be generated. Lumos will prepare a topographic survey map using a combination of aerial photogrammetry and ground collected data. The project will be flown at an appropriate elevation to obtain a horizontal scale of 1"=40' with a 1' contour interval accuracy. The aerial effort will be followed with an on-ground survey utilizing conventional total station equipment to locate detail features not captured by the aerial photography. The on-ground survey will specifically include individual trees above 8"dbh, utility features, and any other features deemed necessary to meet the requirements of the survey. All survey control will be based upon the California State Plane Coordinate System, Zone 2, ground coordinates with elevations tied to the NAVD 88 datum. EXHIBIT A EXHIBIT A October 9, 2024 Page 2 Lumos will review record right-of-way mapping along the project alignment. The mapping for the entire alignment to be surveyed will be calculated prior to beginning the field work. During the ground survey portion of this project, existing right-of-way monuments and property corners will be located along the alignment corridor. A best fit of the existing right-of-way will be made based upon found monuments and record mapping. Once the field data has been collected, mapping will consist of combining the photogrammetry with the ground-based survey data into a single product prepared in the AutoCAD Civi13D platform. The elements of the survey and map will be prepared in compliance with the specified items delineated in the Request for Proposal (RFP) dated August 22, 2024, also attached hereto for reference. Deliverables will include a signed PDF and CAD files (in STPUD format), including an Autodesk Civil 3D surface with appropriate breaklines, and a georeferenced orthorectifed aerial photo, to the specifications listed in the RFP. Additionally, Lumos will provide the other deliverables specified including a grid version of the DWG, survey control points, record documents procured in survey research, and invert sheets for the requested structures. Schedule— Based upon the anticipated NTP date of October 14th, the field work for the Park Avenue portion will be completed prior to the requested date of November 15, 2024, weather permitting. The mapping and deliverables for the Park Avenue project area will be delivered by January 15, 2025. Additional Considerations: • This scope of services does not include any verification or research of existing ownership. There may be areas discovered that could have a material discrepancy in the public record compared to what is found in the field. If this occurs, we will present the findings. If the proposed improvements are in the vicinity of any possible conflicts, additional field work and research could be indicated with the possible requirement of a Record of Survey. In such an event, we would appreciate the opportunity to provide a proposal for a modification to our scope and fees accordingly. • This scope of services does not include any work to prepare easements (if required) for the new waterline sections that fall outside of existing public right of way. • This scope of work does not include monument preservation documentation, typically corner records, filed with the El Dorado County Surveyor. • It has been our experience that the Underground Service Alert will not respond to requests by surveyors for mapping purposes. As such, we cannot guarantee utility locates at the time of survey. If full utility locates are required, Lumos would be happy to prepare an additional proposal to procure utility locates for the project locations prior to the field surveying. • This scope of work excludes any field work that would require traffic control and associated Cal Trans permitting in Highway 50. Two of the projects are also expected to install new water mains in alignments outside of the ROW where there are currently no water facilities. The intent of the survey is to capture topography and existing features information needed for: waterline design in residential and commercial neighborhoods, in ground coordinates, showing the following information: • Horizontal Coordinates based on NAD83 California State Plane Zone 2 (US Feet) • Vertical Control based on NAVD88 (US Feet) •Provide survey point ASCII file in PNEZD comma delimited format. • Provide 3D tin lines (DT ) for surface creation. • Street Address's for parcels on both sides of the subject streets; • Show existing features within and extending 5' behind the ROW along all streets within the Project Area, and the full width of the identified Project Area where it falls outside the ROW, including. o Sidewalk o Curb and gutter EXHIBIT EXHIBIT A Page 2 August 22, 2024 o Center and fog lines o Trees (8" dbh or greater, identify any protected trees) o Bushes, brush and vegetation areas (50 ft2 or greater) o Bear boxes/trash enclosures o Boulders (include approximate diameter) o Rock slope protection o Fences o Gates o Edge of pavement(including roadway and driveways) o Dirt trails and all visible above ground surface features o All visible utility features (gas, electric, communications, storm, water and sewer) o Walls, identifying type o Signs o Mow strips o Guard rails o Any other surface features not listed that would impact the ability to construct underground and surface facilities • Provide rim and all invert elevations (including drops) of sewer and storm drains Size and material type in all manholes, catch basins and inlets. Provide a list of control points in an editable format(.csv or Excel) • Survey any protected areas delineated by District or its consultants within the project boundaries, including but not limited to; national forests, natural bird habitats, protected animal and insect species, protected wetlands, etc. • Work with District to identify within the project area recorded monuments that might be disturbed by project construction, and survey those monuments. • Survey any found Monuments• Topographic survey to extend up to survey limits provided. • Any additional requirements listed in the RFQ for On-Call Land Surveying and Other Geornatics Services Surveyor must replace, as required by law, any monuments destroyed during its survey activities, at no additional cost to the District. In addition to providing the land based topographic survey map in ground coordinates, provide a EXHIBIT A EXHIBIT A Page 3 August 22, 2024 separate DWG file of the survey map scaled to grid coordinates to be used for GIS purposes. The surveyor is required to supply to the District records/supporting data that is used to complete the survey. These files should include, but are not limited to; final maps, parcel maps, tract maps, records of survey, lot line adjustments, assessor parcel maps, utility maps, etc. The surveyor is expected to attend a kick-off meeting with District staff for each project area. These meetings will take place in the field, at the project location, to discuss survey and project details and clarify survey expectations. Surveyor will provide preliminary deliverables using the District-supplied, 2024 STPUD CAD Survey-Mapping.dwt, template, which maps out all necessary layers, colors, line types, blocks, text styles and the ctb, used by the District. Files shall be in DWG and PDF format for review by STPUD prior to submitting final deliverables. The PDF deliverable only needs to be completed for the ground coordinate version of the survey. Final PDF file shall be signed and stamped. The AutoCAD .DWG shall utilize standardized STPUD layering, To ensure utility markings are in place at time of survey, surveyor shall notify Underground Service Alert—Planning and Design in advance of commencing field work. District staff will obtain Right-of-Entry for survey for alignments outside the ROW. It is the District's intent to contract with one or more of the consultants on our current On-Call Surveyors list to complete the work in the three areas. To be considered,please provide a letter proposal describing of the proposed methodology for the topographic survey and an estimate of labor hours,broken down by project area. Please identify key staff who will be assigned to the project, The District will refer to the Statement of Qualifications (SOQ) you submitted previously for supplemental information regarding general firm qualifications. Please provide the fee estimate in a separate file from the proposal. Schedule will be considered in selecting a surveyor and the District would like to have the field survey work for Park Ave area complete by November 15, 2024; the field survey for Wildwood and Gardner Mountain areas may continue into 2025, with final deliverables for all three project areas provided by end of June 2025. If you feel that additional information above and beyond what is requested is necessary for consideration, you may include supplemental documentation as an appendix to your letter proposal, but it will not necessarily be considered.
