TASK ORDER NO. 12
CONSULTING SERVICES
This Task Order is made and entered into as of the 24th day of February 2022 by and
between South Tahoe Public Utility District, hereinafter referred to as "DISTRICT" and
Lumos and Associates, Inc., hereinafter referred to as "CONSULTANT".
In consideration of the mutual promises, covenants, and conditions as addressed in the
Master Agreement dated November 20, 2014, and as specifically hereinafter set forth, the
parties do hereby agree as follows:
1 PURPOSE
The purpose of this Task Order is to authorize and direct CONSULTANT to provide
survey services in support of the vibration assessment at DISTRICT's Luther Pass
Pump Station located in the County of El Dorado, California.
2. SCOPE OF WORK
The scope of work for these consulting services will include providing spot
elevations inside the station using a digital level accurate to 100th of a foot, at
locations shown on Exhibit "A"; performing scans of the interior of the station and
development of a point cloud in Autocad; and providing editable digital files of
coordinates for spot elevations and point cloud. The rate schedule to be used in the
performance of this work is attached hereto as Exhibit "B". Both Exhibit "A" and
Exhibit"B"are incorporated herein by this reference and as such made a part of this
Task Order.
3. STAFFING
Staffing will consist of Greg Phillips, PLS, WRS, Project Manager; and Michael
Craven PLS or other qualified staff as assigned. CONSULTANT shall not change
staffing without approval by DISTRICT. Any changes shall provide for equivalent
qualifications as far as possible and practical.
4. TIME OF PERFORMANCE
Tasks included in this task order will commence on or about February 25, 2022, and
shall be completed on or before March 15, 2022.
5; PAYMENT
Payment to CONSULTANT for services performed under this Task Order shall be as
provided in Article IV of the November 20, 2014, Master Agreement and shall not
exceed the amount of Two Thousand Four Hundred Dollars($2,400.00) unless the
scope of work is changed, and an increase is authorized by DISTRICT.

6. INSURANCE COVERAGE
C SLTT's Errors&Omissions coverage shall be$1,000,000 aggregate with
a deductible of not more than $100,000 for the duration of this Task Order.
7. EFFECTIVE DATE
This Task Order No. 12 will become effective upon signature by all parties and shall
serve as Notice to Proceed upon execution by DISTRICT.
IN WITNESS WHEREOF, this Agreement is executed by the Engineering
Department Manager and Clerk of the Board of the South Tahoe Public Utility District, and
CONSULTANT has caused this Agreement to be executed the day and year first above
written.

• Overtime hours will be billed at 1.5 times standard rate where applicable.
• Survey and Field crew billing rates include standard field survey equipment and truck up to 30 mile radius,after which mileage rates
apply
• Fees for depositions and testimony will be billed at two(2)times the standard billing rates
