TASK ORDER NO. 11
CONSULTING SERVICES
This Task Order is made and entered into as of the 9th day of November 2021 by and
between South Tahoe Public Utility District, hereinafter referred to as "DISTRICT" and
Lumos and Associates, Inc., hereinafter referred to as "CONSULTANT".
In consideration of the mutual promises, covenants, and conditions as addressed in the
Master Agreement dated November 20, 2014, and as specifically hereinafter set forth, the
parties do hereby agree as follows:
1. PURPOSE
The purpose of this Task Order is to authorize and direct CONSULTANT to provide
land surveying services to conduct four Plan and Profile Section Surveys of
DISTRICT'S sewer pipeline crossing at Tallac Creek located in the County El
Dorado, California.
2. SCOPE OF WORK
The scope of work for these services is contained in CONSULTANT's written
proposal dated November 5, 2021. Said written proposal is attached hereto, marked
as Exhibit"A", and incorporated herein by this reference as part of this Task Order.
3. STAFFING
Rick Byrem, P.L.S., WRS, Project Manager shall be the responsible person in
charge of the completion of the services performed under this Task Order.
CONSULTANT shall not change staffing without approval by DISTRICT. Any
changes shall provide for equivalent qualifications as far as possible and practical.
4. TIME OF PERFORMANCE
Tasks included in this task order shall commence on November 9, 2021, and shall
be completed on or before December 15, 2022.
5. PAYMENT
Payment to CONSULTANT for services performed under this Task Order shall be as
provided in Article IV of the November 20, 2014, Master Agreement and shall not
exceed the amount of Eight Thousand Three Hundred Dollars ($8,300.00) unless
the scope of work is changed, and an increase is authorized by DISTRICT.
6. INSURANCE COVERAGE
CONSULTANT's Errors&Omissions coverage shall be$1,000,000 aggregate with
a deductible of not more than $100,000 for the duration of this Task Order.

7. EFFECTIVE DATE
This Task Order No. 11 will become effective upon execution by both parties and
shall serve as Notice to Proceed upon execution by DISTRICT.
IN WITNESS WHEREOF,this Agreement is executed by th Maltagerand
Clerk of the Board of the South Tahoe Public Utility District, and CONSULTANT has
caused this Agreement to be executed the day and year first above written.

Lumos and Associates, Inca is pleased to provide you with this proposal for land surveying services
to compile stream cross-section and profile data at the sewer pipeline crossing at Tallac Creek.

The proposed project is within the service area of the South Tahoe Public Utility District (STPUD) in
the South Lake Tahoe area of El Dorado County, California. It is our understanding that the
purpose of this project is to continue the monitoring of cross-section and profile data surveys
initially conducted by Insite Land Surveys, Inc., at four existing locations. We propose the following
Scope of Services for the project.

Task 1 - Control Recovery&InitialObservations
Lumos and Associates, Inc. (LUMOS) will perform a field survey to recover and verify the location
of the existing control points last referenced by Insite Land Surveys, Inc. in July of 2016 per the
attached exhibit from Insite. Upon completion of the control recovery, a set of data will be collected
for the four existing section lines A-A', B-B', C-C'and D-D'.
All survey data for this project will be reported in California State Plane Coordinates, Zone 2, 1983
NAD horizontal datum and NAVD88 vertical datum. LUMOS will add the new survey data to the
District's Crossing Profile workbook to monitor any profile changes. Changes in profiles exceeding
five feet of lateral stream bank erosion or 2 feet of vertical change in stream bed elevation will be
flagged for reporting by the District to LRQCB.
Task 2— Stream Cross-Section Monitoring
LUMOS will perform subsequent observations of the four established section lines, as stated above,
at intervals of approximately 120 days and at the direction and approval of the District. All methods
and specifications stated under Task 1 above will be followed for the subsequent observations.

Schedule— Upon acceptance of this proposal, LU OS will complete the control recovery and initial
observations within 2 weeks of your notice to proceed, weather permitting..
All work will be performed under the direction of a California licensed Professional Land Surveyor,
license number L- 12 .
