TASK ORDER NO. 10
CONSULTING SERVICES
This Task Order is made and entered into as of the 15th day of October 2021 by and
between South Tahoe Public Utility District, hereinafter referred to as "DISTRICT" and
Lumos and Associates, Inc., hereinafter referred to as "CONSULTANT".
In consideration of the mutual promises, covenants, and conditions as addressed in the
Master Agreement dated November 20, 2014, and as specifically hereinafter set forth, the
parties do hereby agree as follows:
1. PURPOSE
The purpose of this Task Order is to authorize and direct CONSULTANT to provide
land surveying services to replace property corner monuments that have been
distributed or removed by DISTRICT's Water Meter Installation Project, Phases 1-
513, located in the City of South Lake Tahoe, County of El Dorado, California.
Z SCOPE OF WORK
The scope of work for these services is contained in CONSULTANT's written
proposal dated September 21, 2021. Said written proposal is attached hereto,
marked as Exhibit"A", and incorporated herein by this reference as part of this Task
Order.
3. STAFFING
Rick Byrem, P.L.S., WRS, Project Manager shall be the responsible person in
charge of the completion of the services performed under this Task Order.
CONSULTANT shall not change staffing without approval by DISTRICT. Any
changes shall provide for equivalent qualifications as far as possible and practical.
4: TIME OF PERFORMANCE
Tasks included in this task order shall commence on October 15, 2021 and shall be
completed on or before December 31, 2021.
5. PAYMENT
Payment to CONSULTANT for services performed under this Task Order shall be as
provided in Article IV of the November 20, 2014, Master Agreement and shall not
exceed the amount of Four Thousand Dollars ($4,000.00) unless the scope of work
is changed, and an increase is authorized by DISTRICT.
6.: INSURANCE COVERAGE
CONSULTANT's Errors&Omissions coverage shall be$1,000,000 aggregate with
a deductible of not more than $100,000 for the duration of this Task Order.

7. EFFECTIVE DATE
This Task Order No. 10 will become effective upon execution by both parties and
shall serve as Notice to Proceed upon execution by DISTRICT.
IN WITNESS WHEREOF, this Agreement is executed by the General Manager and
Clerk of the Board of the South Tahoe Public Utility District, and CONSULTANT has
caused this Agreement to be executed the day and year first above written.

Dear Jason:
Lumos and Associates, Inca is pleased to provide you with this proposal for land surveying services
to replace multiple property corner monuments that have been disturbed or removed by the
District's water meter project.

The proposed project is within the service area of the South Tahoe Public Utility District (STPUD) in
the South Lake Tahoe area of El Dorado County, California. It is our understanding that the scope
of work for this project will be to ultimately replace as many as 163 such property corners currently
identified by STPUD as needing replacement. We propose the following Scope of Services for the
project.

Property Corner Replacement-Group 1
Lumos and Associates, Inc. (LUMOS) will perform a field survey to replace five specific property
corners along Skyline Drive in Tahoe Paradise Unit No. 14. The corners to be replaced are identified
on the spreadsheet provided by STPUD as having APN numbers 081072014, 081072011,
081072005, 081083002 and 081081003. One corner at each location, as specified on said
spreadsheet, will be replaced and monumented in accordance with the requirements of the
California Land Surveyors Act.
Upon completion of the field work, a single Record of Survey drawing will be prepared for review
by the El Dorado County Surveyor and for recording with the El Dorado Recorder to allow for the
most accurate record of each individual property corner replacement.
These initial corners have been identified to serve as a "pilot"project for the replacement of these
specific corners only. Upon completion of the initial project, review by both LUMOS and STPUD will
occur to evaluate the cost and the procedures involved for the purpose of addressing additional
work.

Schedule Based upon our current workload, LUMOS will be able to complete the field survey,
including placement of the new lot corners, no later than Friday, November 5, 2021. Submittal of
the final Record of Survey to the El Dorado County Surveyor for review will occur within
approximately 30 days thereafter.
All work will be performed under the direction of a California licensed Professional Land Surveyor,
license number L-9129.
